Online registration is the quickest and easiest way to register with the practice as registrations will be processed within 2 working days providing all information is completed. Start the registration process.
If you would prefer to register with the practice using a paper form this can be obtained from the surgery reception.
The information on the medical card will speed up the process of getting your records from your previous practice. Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
When you register with the practice, we will ask you to make an appointment to see our Practice Nurse for a new patient health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.
This form is to be completed by prospective patients
Complete all sections otherwise we will be unable to process your request.
Find your NHS number
You do not need to know your NHS number to use NHS services, but it can be useful to have it
Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter - reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.
